A tile roof works in two layers. The clay or concrete tiles on top shed most of the water, but the underlayment beneath them, along with the flashing at valleys, walls and penetrations, is what keeps water out of the house.
That's why a tile roof can leak even when the tiles look fine. Cracked, slipped or broken tiles can usually be replaced one at a time if matching tiles are available, and flashing problems can be reached by lifting the tiles around them. When the underlayment has worn out across the roof, the tiles generally have to come up so it can be replaced, and depending on their condition they're either reset or the roof is replaced.
